Payment and registration for the issuance of a certificate of expertise
Towards the nurse practitioner licensure examination, those eligible for taking the exam will receive an email from the Department for Professional Training, which will include the information on the exam payment and the issuance of a certificate, including a detailed questionnaire. The email will include full information on how to pay, the proof of payment, and a summons letter for the exam, which will be sent after checking that the payment has been completed.
You should submit 535 NIS for the exam-handling fee and for the issuance of the certificate.
Payment is to be made using the Postal Bank or via a bank wire to the Nursing Division account. The account details: The Postal Bank, branch no. 001, Jerusalem, account no. 0-025961.
Once the payment is complete, scan or photocopy the proof of payment, and send it with the questionnaire to the following email address:
Siud-pituah@moh.gov.il.
Upon receipt of the requested documents and towards the exam schedule, the examination department will send you a summons letter for the exam.
Registration will not be possible without receiving the proof of payment and the complete questionnaire.
